PHASE Scientific International Limited and/or its affiliates (hereinafter called “We”, “Our Company” or “PSI”) is committed to protecting your privacy. This Statement of Privacy Policy (“Privacy Policy”) applies to services provided under PSI’s web-based platform for INDICAID® Lab booking and registration system and INDICAID® COVID-19 RT-PCR Test Kit (“Services”) and governs personal data collection and usage.

 

Please read this Privacy Policy carefully. By using the Services or booking the Services, you consent to the collection, use, disclosure of personal data and other descriptions in the Privacy Policy.

 

  1. Collection of your Personal Information

 

PSI collects personally identifiable information, such as your e-mail address, name, photo, telephone number, citizen identification number including Hong Kong identity card, passport number, geographical location including address for sample pick up service and/or health and wellness-related data including health test samples and results. PSI also collects anonymous demographic information, which is not unique to you, such as your ZIP code, age, gender, nationality, birthday, preferences, interests and favorites.

 

There is also information about your computer or portable device hardware and software that is automatically collected by PSI. This information can include: your website usage information, IP address, device ID, browser type, domain names, access times, referring website addresses and Google Analytics. This information is used by PSI for the operation of the Services, to maintain quality of the Services, and to provide general statistics regarding use of the Services.

 

In order to book for the Services, PSI will collect your full name, telephone number and email address, which are specifically and voluntarily provided by you to proceed to submit the booking and payment on third-party payment system. PSI will store and maintain your personally identifiable information that is provided to verify your identity upon your arrival at the designated place of PSI for the Services. If the booking is made on behalf of you, the person making the booking should have obtained your proper consent before supplying your personal identifiable information for booking. Upon your arrival at the location designated by PSI and prior to perform the test, PSI will collect your personally identifiable information, including identity card number, travel document number, date of birth, phone number, address, email address (where applicable), in order to complete your registration process. Such personally identifiable information will be checked against information provided by you or the person under your proper consent at the time of booking the Services. PSI will also store and maintain your personally identifiable information for future record.

 

Please keep in mind that if you directly disclose your personally identifiable information or personally sensitive data through public message boards, discussion forums or other places provided by the Services, this information may be collected and used by others.

 

  1. Use of your Personal Information

 

PSI collects and uses your personally identifiable information to identify the users, respond to users’ requests or inquiries, provide user with the Services, deliver the Services you have requested and operate, evaluate and improve our business, including but not limited to product development, web development, communications, data analytics, accounting, auditing and research.

 

PSI may also contact you via surveys to conduct research about your opinion of current services or potential new or better products and/or services that may be offered by PSI.

 

PSI may make record of the websites and pages our customers visit when using services provided by PSI, in order to determine what PSI services are the most popular among PSI’s customers or users.

 

PSI uses your personally identifiable information for the purposes stated at the time of collection.

 

PSI will also use your personally identifiable information and history of receiving Services to send you SMS reminders for the up-coming appointments of Services and also to send out lab test report to you through email and/or SMS.

PSI may share your personally identifiable information with our affiliates, business partners, service providers, other parties in connection with corporate transaction or other parties with your consent to help us to perform our business in relation to the provision of the Services or statistical analysis, send you email or postal mail, provide customer support, or arrange for deliveries. All such third parties are required to maintain the same level of protection of personally identifiable information as stated in this Privacy Policy.

 

PSI does not sell, rent or lease its customer lists to third parties without your explicit consent.

 

PSI does not use or disclose sensitive personal information, such as race, religion, or political affiliations, except as required by the applicable law or your explicit consent is obtained.

 

PSI may provide your personally identifiable information to other entities within group of companies of PSI for planning and providing Services to you. By accessing the third-party electronic platforms during the booking process, the personally identifiable information provided by you may be transferred to such third-party service providers for the purpose of completing the booking, system maintenance, or directly related purposes. Personally identifiable information may also be disclosed to law enforcement, regulatory, or other government agencies, or to other third parties, in each case to comply with legal or regulatory obligations or requests. By voluntarily providing information through online booking platform, you are consenting to the disclosures described above.

 

  1. Use of Cookies

 

PSI may use Cookies to improve its website and enhance your online experience. Cookie refers to small text files stored on your device to facilitate us to track your activities in the website. Cookies may be used to remember your preferences when you visit the website and enable us to personalize the information shown in the website according to your needs. At any time, you can disable cookies by changing the settings of your web browser.

To the extent permitted by applicable laws and regulations on data privacy protection, PSI may also use your personal identifiable information for direct marketing purposes, or to send you materials and information on medical and health issues, or communications and notifications regarding services provided by PSI. PSI may contact you by telephone, SMS and text / picture / video message where necessary.

 

PSI may use your name, mailing address, and email address to send promotional materials to you and conduct direct marketing activities in relation to the Services offered by PSI with your consent.

  1. Security of your Personal Data

 

PSI will use its reasonable commercial endeavours to maintain in place security measures and monitoring procedures to protect your personal data and prevent unauthorized or accidental access, processing, erasure, loss or use of the data. All personnel that may access to the data will be ensured complying with the requirements of data protection.

 

When PSI will use our reasonable commercial endeavours to protect your personal data, it is your obligation to use your smartphone with appropriate security measure at all time and review the permissions granted from time to time in order to safeguard against any forms of malware attacks and data breaches.

 

  1. Data Retention Policy

 

PSI will retain your personal data as long as we need it for providing the Services to you and for a reasonable time after termination of the Services for purpose including but not limited to compliance with the applicable laws, regulations and regulatory requirements or rules of Hong Kong. Unless otherwise requested by you or required or permitted by the applicable law, we will retain your personal data for no more than 24 months following account deletion.

The Privacy Policy may be updated from time to time for any reason including customers’ feedback. PSI will post the updated Privacy Policy here and encourages you to periodically review this Privacy Policy.

In the event of any inconsistencies or discrepancies between the English version and any other translated versions of this Privacy Policy, the English version shall prevail.
